New Patient Registration

  • Register with the Practice online

Patients are registered with the surgery, not necessarily with a named doctor. Patients are able to see any doctor of their choice. Whilst the practice will endeavour to comply with a request to see a named doctor it might not always be possible. If this is the case an explanation will be offered. Patients can obtain New Patient registration forms from the surgery.

Practice boundary information

New patients are offered a new patient check, especially for those on regular medication.

You do not need to know your NHS number to use NHS services, but it can be useful to have it

Named Accountable GP information

Register with a GP surgery using a paper form

Complete the paper form to register with a GP.

Print and fill in the form and return it to the practice you want to register with.

You can also get a copy of this form from the practice.

Download the register with a GP surgery paper form and take it to the surgery you want to register with.
